Welcome to The Continuum!

The Continuum is a community-based intergenerational health and wellness center serving clients “From Pediatrics to Geriatrics”.

Serving the community since 1993, our Staff of highly-trained and dedicated professionals represents various disciplines ranging from rehabilitation to home safety solutions to caregiver wellness education and training. This collective of talent assembled within our facility works together as a team to provide a wealth of treatment options focused on total wellness for all ages.

In addition to treatments and training, The Continuum offers Adult and PreSchool Day Programs within a warm and nurturing environment, which we call “intergenerational”. Our members call it “like family”. Through weekly interdisciplinary staffings, family conferences, and close communication with physicians and referral agencies, we strive to provide the highest quality of life by enhancing independence while encouraging individuals to remain active in the home, family and community.
